The Senior Farmers’ Market Nutrition Program (SFMNP) operates in 81 counties! Participants receive benefits to be redeemed for $50 worth of Ohio-grown fresh produce. You are eligible for this program if you are 60 years of age or older AND have an annual household income that is 185% of the poverty level or less.

A proxy is allowed to bring in a completed form on your behalf.

Please bring in your completed application on Thursday, June 16th.

Lifecare Alliance will begin accepting applications at 9:00 am in Banquet Room A. A line will be allowed to form before 9:00 am. However, the building does not open until 8:00 am, and banquet room doors will remain closed until the Life Care Alliance Staff are ready to process applications.
